Completing the MU2 Individual Form on Behalf of a Company Representative

Learn how to complete the MU2 Individual Form on behalf of a company representative (control person) to ensure their information is accurately prepared in NMLS before requesting attestation.

About this task

The MU2 Individual Form is required for each company representative identified in a Company (MU1) filing as a direct owner, indirect owner with control, executive officer, or qualifying individual, or in a Branch (MU3) filing as a branch manager. These forms collect personal, employment, and regulatory information needed for licensing and must be attested by the individual before the MU1 or MU3 can be submitted.

Company users may complete some or all sections of the MU2 form on behalf of the individual, then request their attestation in NMLS. The individual can also log in to complete portions of their form prior to attesting.

Procedure

  1. Navigate to the NMLS Login page (State Context).
  2. Log in to your NMLS account.
  3. Access the Filing tab and select Company (MU1) or Branch (MU3).
  4. Create or edit a filing, then open the MU2 Forms section from the left navigation panel.
  5. Click the Edit Filing icon next to the company representative’s name to open their MU2 form.
    Note: If the MU2 form has already been attested, use the Recall Filing icon to make it editable.
  6. Complete applicable sections of the MU2 form, including personal information, employment history, disclosure questions, and other sections similar to MU4 filings.
  7. Request a credit report and initiate a criminal background check (CBC) if required, using the Credit Report and Criminal Background Check links in the left navigation.
    Note: Refer to state-specific requirements in the License Requirements and Fees Chart to confirm credit report and CBC needs.
  8. Upload any required supporting documents using the Document Uploads section.
  9. When ready, click Attest and Submit in the left navigation and select Request Attestation to send the form to the company representative for their review and attestation.

What to do next

Once the MU2 individual attests to their form, you will receive an email notification. Return to your MU1 or MU3 filing to complete submission.
