Completing an Individual MU4 Filing
Learn how to complete and submit an Individual MU4 filing to apply for a license or registration through NMLS.
Before you begin
About this task
If you are applying for a license or registration in NMLS, you must complete an Individual (MU4) Filing. This form collects your identifying, residential, employment, and disclosure history and allows you to authorize background checks and select license types.
Procedure
- Navigate to the NMLS Login page (State Context).
- Log in to your NMLS account.

Click the Add New License button in the My
Licenses section or the MU4 Filing link under
My Tasks if you already have a filing in progress.
An MU4 Form is opened.
- Search for the desired license or click Browse All Licenses and select desired license from the list.

Complete all required sections of the MU4 Form using the links on the left
navigation panel.
Note: Review the information in the Agency Guidance section of the MU4 Form to ensure all requirements are met. Click Completeness Check to see errors or warnings in your filing as you complete it. Click Save to save your progress and return to your filing later. Click Delete to abandon the filing.
- Click Attest and Submit to finalize your application. See Attesting to Your Individual MU4 Filing for more information.
- Pay all required fees. These may include filing, background check, education, and state processing fees.
- Track your application status using the My Licenses section on the Home tab of NMLS Connect.
