Individual MU2 Form Filing

The Individual (MU2) Form is required for certain natural persons associated with a company or branch filing in NMLS. The Individual Form (MU2) is an automatic requirement and is part of the Company and Branch Forms. These filings will not be considered complete without all Individual (MU2) data entered and attested to by each individual.

The Individual (MU2) Form is required for all natural persons identified in the Company Form. In addition, all Qualifying Individuals and Branch Managers identified on the Company Form (MU1) must submit an Individual (MU2) Form. Any natural person who is an indirect owner of 10% or more must submit an Individual (MU2) Form. An applicant or licensee must also refer to state licensing requirements published by each state in which it is applying. Some states may require biographical information about individuals that do not fit the Control Person definition. These states may therefore request an Individual (MU2) Form with other filings. Applicants must also update the roster of control persons on the Company Form as needed, potentially requiring additional MU2 filings. State agencies may require authorization for a credit report and/or a criminal background check to be submitted upon filing the Individual (MU2) Form.
