This document type was added in connection with the Portability of Professional Licenses of Servicemembers and their Spouses” amendment to the Servicemembers Civil Relief Act (SCRA).

Document Description(s)

Proof of an individual’s military service and/or transfer orders is required by the federal law. Many states have implemented statutes or regulations to meet the requirements of the SCRA. Some states have additional licensing requirements for current or former military members.

Amendments

Existing document must be removed and replaced by new document. Will not be processed unless old document is removed.

CAC Card (Active Duty), DD Form 214 (Veterans), Transfer Orders

Does Not Include

Any documents that include Personal Identifying Information or sensitive information (i.e., Information Technology Audit), information related to criminal background/history checks, etc.

Any Personally Identifiable Information (PII) should be redacted before being uploaded into NMLS.
