About this task

If your legal name, other names, mailing address, contact information, government identification, or status as a servicemember or veteran has changed, an MU4 amendment may be required.

Procedure

Navigate to the NMLS Login page (State Context).
Log in to your NMLS account.
Select Update Record in the Quick Links section.
The MU4 Form is opened.
Select the Individual Information link on the left navigation panel.
Edit your personal information as needed.
