Applying for a State Company License

This section explains what the Company Form (MU1) is and how to apply for a state license or registration for your company in NMLS.

NMLS Company Form (MU1) must be completed by a company applying for a license/registration in one or more states.

If the company is applying for a new license, the Individual Form (MU2) must also be completed by individuals identified in the Direct Owners & Executive Officers, Indirect Owners with control and Qualifying Individuals on the Company Form (MU1).

The Company Form (MU1) can be used to request licenses in multiple states, however, not all license types are managed in NMLS and some information may have to be submitted to state regulators outside of the System.

The following guidelines are organized to follow the Company Form’s format in the System.
