A-C License or Registration Information A list of the state agencies, license names, and license requirement statutes.
D-G & K Branch Manager Criminal Background Check (CBC) and Credit Report Requirements Provides a Yes or No indicator for the Credit Report and conveys what action will require the Branch Manager to complete a CBC.
H-J NMLS Document Uploads Indicates if the documents referenced in the column header must be uploaded in the Document Uploads section of the MU2. A Yes in these columns means the document listed must be uploaded during the new application process.
O-T Fees Provides the fees required for the selected license as well as any additional information needed.
