Completing the MU4 Individual Form on Behalf of an MLO

Follow the steps to complete the MU4 Individual Form in NMLS on behalf of a Mortgage Loan Originator (MLO), ensuring all required information is accurate before submitting for their review and attestation.

About this task

Companies can initiate and complete portions of an Individual (MU4) Form on behalf of a mortgage loan originator (MLO). The MLO must have an NMLS individual account and must ultimately attest to the filing before the company can submit it to the regulator. Procedure

  1. Navigate to the NMLS Login page (State Context).
  2. Log in to your NMLS account.
  3. Select the NMLS Connect tab.
    You are navigated to the NMLS Connect home screen.
  4. Select the Manage People icon on the left navigation panel.
  5. Search for the desired individual and Select the hyperlinked Individual Name.
  6. Select Add New License in the Actionssection.
  7. Complete required sections in the left navigation panel for the MU4 filing.
  8. When ready to involve the MLO, select the Attest and Submit section on the left navigation panel.
  9. Select the Request Attestation button.
    Note: The individual is notified instructing them to attest to the filing.
  10. After the MLO has attested, go to NMLS Connect and select the Requests & Filings icon on the left navigation panel.
  11. Locate the filing in an Attested status and select the MU4 Filing link.
  12. Select the Attest and Submit link in the left navigation panel.
  13. Select I verify that I am the named person above....
  14. Select the Submit Filing button.
  15. Follow any payment prompts, if applicable.

Results

The MU4 Form is submitted and sent to the relevant state agencies for review.
