Chapters II through V explain specific details of each state licensing form: MU1; MU2; MU3; and MU4.
Chapter IV—NMLS Branch Form (MU3)
The Branch Form (MU3) is the branch office information tied to a company’s record. The Branch Forms may only be submitted after a company submits a Company Form (MU1).
MU2 Form
The system requires an Individual Form (MU2) to be completed for each Branch Manager.

- Chapter II—NMLS Company Form (MU1)
  NMLS Company Form (MU1) must be completed by a company applying for a license/registration in one or more states.
- Chapter III—NMLS Individual Form (MU2)
  The Individual Form (MU2) must accompany the Company or Branch Form. Each individual identified as a Control Person, Qualifying Individual, or a Branch Manager for the applicant, must complete the Individual Form (MU2).
- Chapter IV—NMLS Branch Form (MU3)
  The Branch Form (MU3) is the branch office information tied to a company’s record. The Branch Forms may only be submitted after a company submits a Company Form (MU1).
  - General Instructions
    General instructions for filing form MU3, including filing, main in-state branch location, and data shared to NMLS Consumer Access.
  - Create a Branch (MU3) Filing
    The address information listed here must be for the Branch location.
  - Business Activities
    Applicants and licensees should indicate all business activities the branch engages in, regardless of whether those activities are undertaken in a state in which they are applying for a license or whether the state manages certain license types in NMLS.
  - Request a License
    The Request/Transition License(s) screen identifies the license / registration types available based on the business activities (see reference/policyGuide/maps/topics/nmlsPG_bizActivityDef.html for the current business activities list and definitions) and states selected.
  - License / Registration Information
    The License/Registration Information screen will display a list of all license/registration requests pending submission as well as any previously submitted license/registration requests.
  - Identifying Information*
    Physical address, mailing adfress, business phone, fax, and email on the MU3 form.
  - Other Trade Names
    Other Trade Names must be completed for all the other names the company will be using at this branch location.
  - Branch Manager*
    NMLS requires the identification of a Branch Manager. The Branch Manager must have a completed Individual Form (MU2) filed at the same time as the Branch Form (MU3).
  - Web Addresses*
    A Web Address is optional in NMLS. If a web address is provided, applicants must indicate if the company is accepting applications or transacting business through the website.
  - Books and Records Information
    Applicants and licensees must list the company name and address of the location where books and records are stored. They must also list the name of the individual at this location that should be contacted with inquiries about or to gain access to the books and records.
  - Operation Information
    Consult state licensing requirements if a Yes answer to any of these questions requires the submission of items outside the system.
  - Expense Information
    If you answered Yes to the fourth question on the previous screen [Other than the entity, is anyone responsible for the expenses or have a financial interest in the activities of this branch?], you must provide information about the entity here.
  - Advance Change Notice (ACN)
    NMLS allows state licensees to submit certain changes to their NMLS Record in advance of the desired effective date. This functionality permits state regulators to electronically process future NMLS Record amendments and reduces the forms and filings submitted outside the system. State licensing requirements should be consulted as to when items should be submitted.
  - Document Uploads
    The Document Uploads section allows users to upload and submit PDF documents to regulators.
  - Historical Filings
    The Historical Filings section under the Filing tab contains copies of previously submitted Branch filings.
  - MU2 Form
    The system requires an Individual Form (MU2) to be completed for each Branch Manager.
  - Attest and Submit
    The Attest and Submit button for the MU3 Branch Form will only appear if the Company Form (MU1) passes all completeness checks, including Individual Form (MU2) attestations.
- Chapter V—NMLS Individual License Form (MU4)
  The Individual License Form (MU4) is the Individual Mortgage License / Registration & Consent form.

The system requires an Individual Form (MU2) to be completed for each Branch Manager.
