    - Company New License Application
    - Company Amendment
      This information is applicable to companies who have an existing license and wish to make changes to the Legal Name or Status, Main Address, Other Trades Names or Owners and Control Persons.
    - Company Periodic Reporting (MCR, MSBCR, or UAAR)
      This information is applicable to companies who have an existing license and need to satisfy any periodic reporting requirements, including the Mortgage Call Reports (MCR), Money Service Business Call Reports (MSB), and Uniform Authorized Agent Reporting (UAAR).
    - Branch New License Application
    - Branch Amendment
      This information is applicable to companies who have an existing branch license and wish to make changes to their MU3 filing.
    - Individual New License Application
    - Individual Amendment
      There is no information for individual amendments included in the License Requirements and Fees Chart. Review the relevant state-specific checklists using the State Licensing Checklist Compiler to ensure that you have all information and requirements.
  - Using the Checklist Compiler
    Use the Checklist Compiler on the NMLS Resource Center to find and export state licensing checklists that outline what requirements you must complete for an NMLS filing (including tasks inside and outside NMLS).

This information is applicable to companies who have an existing license and wish to make changes to the Legal Name or Status, Main Address, Other Trades Names or Owners and Control Persons.

Columns	Category of Information	Notes
A-C	License or Registration Information	A list of the state agencies, license names, and license requirement statutes.
S-Z	Number of Days in Advance the Change Must be Reported	Indicates the number of days in advance the change needs to be reported for an Advance Change Notice (ACN).
BB-BI	Fees	Provides the fees required for the selected license as well as any additional information needed.
